Injected provider becomes stale after extension update or reinstall — wallet popup doesn't appear

Summary

After updating or reinstalling the Phantom extension, the injected window.phantom / window.solana provider in already-open tabs becomes stale. Calling open() via Reown AppKit silently does nothing — no popup, no error. The user has to manually reload the page.

Example

Reproducible on any dApp using Reown AppKit (@reown/appkit) with SolanaAdapter for wallet connection.

Steps to Reproduce

1. Open a dApp that uses Reown AppKit with Phantom for Solana wallet connection
2. Remove and reinstall the Phantom extension (or let it auto-update)
3. Without reloading the tab, click the "Connect Wallet" button
4. open() from useAppKit() is called but no popup appears
5. No error is thrown — it fails silently
